Gift voucher

TwojeZnamiona.pl sells gift vouchers that entitle the holder to a videodermoscopy service under these terms and conditions.

This is a translation provided for information. The Polish version of these terms is the binding one.

For the purposes of these terms and conditions, the following definitions apply:

• Gift Voucher — a personalised document entitling the holder to a videodermoscopic mole examination.
• Customer — the person who purchases the gift voucher.
• Gift Voucher Issuer — Medify.me spółka z ograniczoną odpowiedzialnością, with its registered office at ul. Jagiellońska 12, 84-200 Kąpino, Poland, tax identification number (NIP) 7773357647, owner of the TwojeZnamiona.pl brand and operator of a nationwide network of mole screening centres.

Conditions of use

1. The voucher is personalised. The recipient's first name and surname are entered on it at the time of purchase, and only that person may use it. The voucher cannot be transferred to anyone else.
2. The validity period is shown on the gift voucher and is 12 months from the date of purchase. For online purchases, the date of purchase is the date the payment is credited.
3. If the value of the gift voucher was equal to the cost of an appointment at the selected centre on the date of purchase, the price from that date will continue to apply for 12 months. The voucher can therefore be redeemed without an additional payment if prices change during that period.
4. The gift voucher expires if it is not used within the validity period shown on it.
5. The gift voucher entitles the holder to the services listed in point 2 in the following cities: Poznań, Warszawa, Wrocław, Szczecin, Kraków.
6. If the voucher is lost, the customer may still use the service by providing proof of purchase and the voucher number, and presenting an identity card, passport or driving licence.
7. The gift voucher is not a payment method. It cannot be exchanged for cash or refunded to a payment card.
8. Only an adult may purchase a gift voucher. If the gift voucher is bought for a child, the child must attend the appointment with their legal guardian. Otherwise the service cannot be provided.

Conditions of redemption

1. To redeem a gift voucher, present the original printed voucher if it was bought at a centre, or the electronic voucher if it was bought online. You must also show an identity document so that the details can be verified.
2. To arrange an appointment, call the selected centre, book online at www.twojeznamiona.pl or email zbadamy@twojeznamiona.pl. When booking the videodermoscopic examination, state that you will be using a gift voucher.
3. If you cannot attend the agreed appointment, contact us by phone or e-mail at least 24 hours beforehand so that a new date can be arranged.
4. The Issuer reserves the right to refuse the videodermoscopic mole examination against a gift voucher if the voucher is damaged or illegible, or if there are reasonable doubts as to its authenticity.

Final provisions

1. If the gift voucher cannot be redeemed at the chosen location through the fault of the Issuer (for example, if the TwojeZnamiona.pl clinic in that town closes), the Issuer will extend the validity of the voucher by a further 6 months and make it possible to redeem it in another town where TwojeZnamiona.pl has a clinic accepting gift vouchers.
2. By purchasing a gift voucher, the Buyer confirms that they have read and accepted these terms in full.
3. The Issuer reserves the right to amend these gift voucher terms without notice. Customers will be informed of any changes through the updated terms and conditions published at twojeznamiona.pl.
4. These terms and conditions came into force on 1 January 2023 and apply for as long as gift vouchers remain on sale.
